 Prior to Your Appointment

Please be certain of the following:

  1. The identification document you will be using to prove your identity is one the above and is not expired.  The document must be an original and not a photocopy.  You may not use a temporary license or ID card issued by DMV.  Temporary license and ID cards do not contain an original photo on.
  2. The name you will be signing on the document must match the name shown on the documentation you are using to prove your ID.  An ID in your maiden name along with a marriage license evidencing your married name is not acceptable.  In the event the names on your ID and the document being signed do not match, please be prepared to have two credible witnesses available to identify you.  Your credible witnesses must have proper ID as described above.
  3. The document to be notarized is complete.  If there are blanks in the document, please fill them in prior to your appointment.  If you have questions about how to complete the document, contact the person who drafted or requested the document.
  4. Signer must be able to communicate with the notary public.  If the signer is a non-English speaking individual who does not fluently speak English, please find a notary who speaks your language.  If you need help locating a Spanish-speaking notary, please complete the Request an Appointment or Information form below.  I will be able to provide you with the name and contact information of a Spanish-speaking notary.
